ASK THE DOCTOR

I have been diagnosed as clinically depressed, and I recently started taking an SSRI
antidepressant. I'm feeling great otherwise, but my interest in sex has disappeared... is there any way to reverse this?

I assume this is asking about an SSRI side-effect, but want to make certain. My first question
would be whether the sexual dysfunction existed before a patient began medication, as this is
often a symptom of depression itself.  In that case, an antidepressant may help restore sexuality,
but, it is usually the last symptom to be reversed. Someone who has just started an SSRI may
want to give it some more time before taking further action.  I would also determine whether the
sexual dysfunction is due to substance abuse or other health problems.

If it indeed appears to be a side affect of the medication, I'd discuss the options with the patient,
who would then choose what s/he wants. As side effects sometimes reduce in time, s/he may
wish to wait for a few weeks more and see if that's the case. We could also try lowering the
dosage to find a still-therapeutic level, but with lesser side effect.

Another possibility  is to switch to another class of antidepressant, which may improve sexual
function. The patient must be aware that s/he may have a recurrence of the depression - the other
drug may not work for hir, or may not work immediately. We can however always reinstate the
effective drug.

S/he may prefer to try adding another medication. There are some reports that the herb gingko
biloba is effective. I would caution here that like other herbs, it may interact dangerously with
your existing medication. Consult your doctor before adding anything to your regimen. There are
also early studies that indicate that sildenafil (Viagra) can restore orgasmic capacity and sexual
interest in both women and men who are experiencing sexually-discouraging side effects of
antidepressants.

Not only SSRI's, but certain of the older antidepressants can sometimes decrease sexual desire
and/or inhibit orgasm. These side effects often contribute to a patient's quitting medication.  I
make a point of asking about it, since it may otherwise go unmentioned.
